ELIGIBILITY REQUIREMENTS

GENERAL REQUIREMENTS

1. Applicants must live within the following zip code areas: 30327, 30328, 30338, 30342, 30346, 30350.
2. Applicant must provide the following documentation:

• Current lease agreement. (Adults not on the lease must also provide proof of address.)
• Documentation of all residents living at a given address: for adults: picture ID; for children: school enrollment, birth certificate or Social Security card.
• Social Security card. (For applicant)
• Proof of income.

FINANCIAL ASSISTANCE
Financial assistance is given at most every six months. (You may reapply one month after six months have elapsed.)

1. Applicant must have been a community resident for at least three months (documented).
2. Applicant must provide proof of income by any of the following: Last 3 pay stubs; IRS tax documentation; AFDC / Food stamp letter.
3. Applicant MUST BE ABLE TO PROVIDE PROOF OF FINANCIAL NEED (recent not ongoing). (Ex. separation notice, medical expense receipt, etc.) Without this, we cannot provide financial assistance. (Documented situation can be used only once to apply for assistance.)
4. CLIENT'S NAME MUST APPEAR ON THE BILL.
